All forms below can also be found in the paperwork section of your child’s account in CampBrain. If you have specific health questions please call 202-537-8133.

Sidwell Summer’s Sunscreen Policy: All campers should bring their own sunscreen labeled with their name for all programs; even indoor-based programs spend some time outside during the day. We offer reminders to re-apply sunscreen over the course of the day and prior to any water play activities. Please make certain that sunscreen has been applied to your child before dropping off each day. We do not directly apply sunscreen to campers; however, we will supervise application.

  • Sunscreen Authorization (Bethesda Campus) - Complete this form if you plan to send your child to camp with sunscreen. The form should be sent along with any sunscreen being brought to camp.

(Potentially) Mandatory Forms:

  • Allergy Action Plan - Required, with physician’s signature, if your child has allergies and an Epi-Pen. Note, you must provide a non-expired, pharmacy labeled Epi-Pen.

  • Asthma Action Plan - Required, with physician’s signature, if your child has asthma and uses an inhaler. Note, you must provide the inhaler in the original pharmacy container.

  • Diabetes Medical Management Plan - Required, with physician’s signature, if your child has diabetes. Note, you must provide all necessary medication and equipment.

  • Immunization Certificate - Mandatory for any camper participating in programs on the Bethesda Campus who lives outside the United States or US Territories. Per State of Maryland Law, camper will not be permitted to attend camp without the completed form on file before the first day of camp.

  • Medication Authorization Form (DC Campus) - Required, with physician’s signature, if your child has medications to be administered during camp hours. Note, you must provide the medication in the original pharmacy container.

  • Medication Authorization Form (Bethesda Campus) - Required, with physician’s signature, if your child has medications to be administered during camp hours. Note, you must provide the medication in the original pharmacy container.

  • Physician's Clearance (Bethesda Campus) - Required, with physician’s signature, if your child is under age 5 and attending camp on the Bethesda Campus.

  • Seizure Action Plan - Required, with physician’s signature, if your child has a history of seizures. Note, you must provide all necessary medication in the original pharmacy container.

Optional Forms:

  • Over-the-Counter Medication Authorization - Physician’s signature required in order to have any over-the-counter medications (Advil, Benadryl, Tylenol, etc.) or ointments (first aid cream, antibiotic, etc.) administered to your child while at Sidwell Summer.

  • Camper Advocate Form - Complete this form if your child has any needs (medical or otherwise) that you would like Sidwell Summer to be aware of and think would help your camper and others have a better camp experience.
